I.V. THERAPY

Dispensary
The supplements in our dispensary undergo extensive quality assurance and quality control testing to ensure they meet the highest standards for safety and efficacy. Most are available by prescription only. We offer all supplements at the best price possible with additional discounts when applicable.
Supplements can also be purchased online through Fullscript, which offers discounts when ordered by a physician. Fullscript will conveniently deliver all prescribed supplements to your home.

Download the Patient Registration Form + Consent Form below before your first visit, fill it out, and send it to us through the portal found top left of this page, via email, or bring it in along with your insurance card and photo ID.
Insurance Information
Our providers are contracted with major insurance companies. For accurate benefits and coverage information, please contact the appropriate insurance company prior to your visit as benefits and coverage vary widely among insurance plans. Once we bill the insurance company as a courtesy for the services you receive, we are notified if there are any co-insurances and/or deductibles for which you are responsible. This will be billed to you at a later date. However, please be prepared to make your co-payment at the time of service as you are required by your insurance company. Our office accepts debit cards, credit cards (Visa, Mastercard), cash, and checks.
